An investigation is underway after a Carlow mum died just minutes after giving birth to her third child at a local hospital. Mother-of-three Tracey Fitzpatrick from Templepeter gave birth to her healthy baby, who the family have named Max, at St Luke's Hospital on Easter Monday. According to reports, the 36-year-old was being taken for surgery after the birth when she passed away moments later. A hospital spokeswoman told The Irish Daily Star: "The staff and management of St Luke's Hospital in Kilkenny are currently focussed on supporting the family at this very sad time.
"An internal review process has commenced with a full investigation to follow."
